• Credit Union Payroll Deduction
  For changes to Credit Union deduction from payroll

• Direct Deposit Authorization Form
  For changes to Direct Deposit. A separate form must be filled out for each account affected.

• OMNI Salary Reduction Agreement
  For changes to Tax Sheltered Annuities. This form may be faxed or mailed directly to the OMNI Group or send the completed form via interoffice mail to Lisa Jayne Doyle in the Payroll Office for submission.
