Committee on Preschool Special Education (CPSE) Referrals

If you suspect your child has a disability, or your child will be aging out of Early Intervention services, there are two steps you must complete to have your child evaluated.

  1. Step One: You will need to complete a CPSE referral.
You can begin the process by clicking here.
  2. Step Two: Once you have completed the CPSE referral form, you will need to register your child with our school district.
View the district registration form here.

Upon completing the pre-registration link you will receive an email from the Registrar with the next steps to finalize your child’s enrollment.

Should you have any questions regarding the CPSE process, please call the Office of Pupil Personnel Services at (845) 615-6450 and select option 1.

Forms

The following documents are required to register a student into the Goshen Central School District:

1. Complete an Online Registration Form

The district follows an efficient online registration process. Please note the following:

  • It takes approximately 20-30 minutes to complete the online registration form.
  • Once you start the online registration form, you must finish it in its entirety – it will not save and you cannot pick up where you left off.
  • The online registration form must be  completed or your child will not be officially registered, thus delaying the process.
  • When the online registration form is complete, you will see a congratulatory screen with the Gladiator logo, so you’ll know you’re done.

Register your child using our online registration form.

If you need assistance or have any questions, please contact Registrar Maria Cepeda at (845) 615-6767 or registrar@gcsny.org.

2. Proof of Residency

The District requires TWO proofs of residency from each list below. Please submit copies. (Note: This is not intended to be an exhaustive list, and the District may consider other documentation and/or information as appropriate):

  1. ONE of the Following
    • For homeowners and renters:
      • A copy of a residential lease or proof of ownership of a home, such as a deed or mortgage statement.
    • For those renting without a lease or living with a District resident:
      • A notarized or signed statement by a third-party landlord owner or tenant from whom the parent(s), guardian(s) or person(s) in parental relation leases or with whom they share property within the District.
  2. And ONE of the following:
    • Pay Stubs
    • Federal or NYS Income Tax, W-2 or Earnings Statement
    • Utility Bill
    • Voter Registration Notification Card
    • Official driver’s license, learner’s permit or non-driver identification
    • Documents issued by federal, state, or local agencies (such as social services agency) or Government issued identification
    • Membership documentation based on residency

3. Child’s Birth Certificate

You must provide your child’s original birth certificate with a raised seal for students entering kindergarten. A copy is sufficient for all other grades. If you do not have an original birth certificate, we will accept your child’s original baptismal certificate with a raised seal. If those are not available, a passport may be used to determine a child’s age.

If none of the above are accessible or available, the following documents are acceptable alternatives as long as they have been in existence for at least two years.

  • Official driver’s license, State or other government issued ID
  • School photo ID with date of birth
  • Consulate ID card
  • Hospital or health records
  • Military dependent ID card
  • Native American tribal document
  • Record(s) from non-profit international aid agencies and voluntary agencies
  • Court order or other court-issued documents
  • Documents issued by Federal, State or Local agencies (e.g. Local Social Service agency, Federal Office of Refugee Resettlement)

4. Health/Immunization Records

Students must have an up-to-date immunization record, which complies with New York State Department of Health requirements. On July 1, 2018, the state has released a new School Health Examination Form that parents and doctors should use. The form is available on the State Education Department website and must be signed or stamped by a licensed healthcare provider.

Note: A physical exam is required for all new students.

5. School Records

Transcripts are requested of all high school students; report cards are recommended for all other grade levels. If your child is classified with a disability, if possible, please submit any supporting documents and reports from the previous district.

6. Custody Documents (if necessary)

Only a person in parental relation is able to complete the registration process below. A person in parental relation is defined as a parent, by birth or adoption, a step-parent, a legally appointed guardian or a custodian. If the student is not living with their parent or legal guardian, please complete the Parent and/or Custodial Affidavit. Please contact the District Registrar with any questions.

If there are special circumstances such as custody agreements or orders of protection, please submit those documents to us. The schools cannot refuse to release a child to a parent/legal guardian unless there are court documents on file with the District to the contrary.

If there are any other special circumstances such as name change, please bring those documents with you also.

7. Parent Driver’s License

Please bring your Driver’s License or ID.

Universal Pre-K Information

The Goshen Central School District Universal Pre-Kindergarten (UPK) Program began in September of the 2021-22 school year after receiving full funding from the New York State Department of Education.

The district partners with On My Way Early Learning and Childcare Center in Middletown to provide a robust curriculum for students in order to prepare them both socially and academically for kindergarten. Students receive instruction in language and literacy, math, social studies, and science that is in line with New York State Common Core Standards. Hands-on learning and small group activities are just a part of the UPK’s exciting curriculum.

The UPK program is housed at Scotchtown Avenue Elementary School in two classrooms (in its inaugural year, the program was housed at On My Way Early Learning and Childcare Center). Currently, there are 54 spots available. Eligible students are selected via a lottery system; details on how to pre-register your child to be entered into the lottery system are shared by the district each spring.

If you have any questions, please contact UPK Director Peggy Fuentes at (845) 692-5437.
